The Johnson County Sheriff’s office has released the results of its investigation into a pair of deaths at Lake Macbride last month.

Ten-year-old Borlaug Elementary School student Ahmed Osman and his teacher, 24-year-old Hannah Jorgensen, died after they ended up in the water during a fifth-grade School of the Wild field trip on September 22nd.

The sheriff’s office says in a Friday afternoon news release that during a stop for lunch, Osman stepped into the water flowing down the spillway and slipped on the rocks, causing him to fall into the water of the Coralville Reservoir. Jorgensen immediately jumped in the water to rescue Osman, leaving her phone on the shore. Once in the water, she directed her students to call 9-1-1. One student used Jorgensen’s phone to make the call while other students alerted a bystander on the other side of the spillway. The bystander also jumped into the water to rescue Osman and Jorgensen but was unsuccessful, though he managed to return to shore safely. The bystander stayed at the scene with the students until first responders arrived and was interviewed. The remaining 13 students were escorted back to the school by staff and all students were later interviewed as part of the investigation.

The incident remains under investigation.
